Getting Started
We look forward to meeting with you during your introductory interview. Before your first appointment, please read and sign the appropriate client forms listed below. Copies of these forms will be provided to you during the introductory interview. Your counselor will review these forms with you during your first appointment and answer any questions you may have about the therapy process.
Insurance & Payment
Wellness Associates offers a monthly invoice to clients upon request for reimbursement purposes. The invoice itemizes the services rendered, service fees, and a diagnosis (if applicable) that can be used by insurance carriers to reimburse a portion of the fees you paid for therapy. In this case, Wellness Associates is considered an “Out of Network” provider (i.e. we are outside of your insurance company’s preferred provider network). Some of the fees for our services may be reimbursable to you directly through your insurance provider. Consult with your insurance company to determine which fees may be reimbursable. We accept cash and all major credit cards (Visa, Master Card, American Express, and Discover). Payment will take place at the end of your appointment.
